Project aims
To work with older people who are at risk of hospital admission and offer practical, timely help and interventions. To encourage activities that each person is passionate about that will promote health and wellbeing in order to prevent hospital admission.
Project description
Our project seeks to prevent avoidable hospital admissions. We will accept referrals from Adult Social Care/Health Professionals and from our services for those in a short-term crisis or managing long-term health conditions. Post-referral, we will assess individuals and work with them to tailor a support program. This could include short-term home care, access to our Day Centre services (with transport), introduction to activity groups and signposting to other services. We will liaise with GPs and other medical professionals to organise further support. We will also conduct a formal needs assessment where longer-term services are needed. Where a project service user is admitted to hospital, the team will work from the point of admission to facilitate rapid discharge. This will be done through prior knowledge of the individual’s wishes and circumstances so we can work with hospital staff to make their hospital stay as short as possible. Our input includes practical tasks e.g. having keys cut, help with moving furniture where special equipment is needed upon discharge. The project will work with our Home from Hospital service, which supports patients after discharge. This will provide emergency food parcels, short-term care and access to counselling and benefits advice.
